El Inglés en la Medicina: Una Competencia Crítica
El inglés es la lingua franca de la medicina global. El 75% de los artículos científicos médicos se publican en inglés, y los congresos internacionales de mayor prestigio — ASCO, AHA, EASD, ESC — se desarrollan íntegramente en inglés. Para un médico o especialista que aspira a la excelencia clínica e investigadora, el inglés médico no es un complemento; es una herramienta de trabajo fundamental.

Historia Clínica (Clinical History)
La estructura de una historia clínica en inglés sigue el formato SOAP:
- Subjective (Subjetivo): Lo que refiere el paciente
- Objective (Objetivo): Hallazgos en la exploración
- Assessment (Valoración): Diagnóstico o diagnósticos diferenciales
- Plan: Plan terapéutico
Ejemplo de nota SOAP:
Subjective: 45-year-old male presents with a 3-day history of productive cough, low-grade fever (38.1°C), and right-sided pleuritic chest pain. No significant past medical history. Non-smoker. No recent travel.
Objective: Temperature 38.2°C, HR 98 bpm, RR 22/min, SpO2 94% on room air. Dull percussion note and decreased breath sounds at right base. CXR: right lower lobe consolidation.
Assessment: Community-acquired pneumonia (CAP), right lower lobe. Low CURB-65 score (1). Outpatient management appropriate.
Plan: Amoxicillin 1g TID for 5 days. Paracetamol PRN for fever and pain. Follow-up in 48h or earlier if symptoms worsen. Advice given: rest, adequate hydration, return if dyspnea increases.

Lenguaje del Abstract
El abstract es la primera (y a veces única) parte que leerá la mayoría de lectores. Debe ser preciso y persuasivo.
Frases estándar por sección:
Introducción/Background:
"[Condition] affects approximately [X] million people worldwide and represents a significant burden on healthcare systems." "Despite advances in [treatment area], [specific problem] remains poorly understood."
Métodos:
"We conducted a prospective, randomized, double-blind, placebo-controlled trial..." "Patients were enrolled between [dates] at [institution(s)]. Inclusion criteria were..."
Resultados:
"A total of [N] patients were enrolled. The primary endpoint was met in [X]% of the intervention group vs. [Y]% in the control group (p < 0.001)." "There was a statistically significant reduction in [outcome] (HR 0.72, 95% CI 0.58–0.89)."
Conclusión:
"These findings suggest that [intervention] is effective and safe for [condition] in [population]." "Further large-scale studies are needed to confirm these results and determine optimal dosing regimens."

Estructura de una Presentación Oral
Los primeros 60 segundos son críticos. Establece por qué tu investigación importa:
"Every 6 seconds, someone in the world is diagnosed with [condition]. Despite decades of research, the 5-year survival rate has barely improved. Today, I want to share with you data from our phase III trial that we believe changes this equation."

Presentando métodos:

La Consulta Estructurada en Inglés
Apertura:
"Good morning. I'm Dr. García. I understand you've been experiencing some [symptoms]. Could you start by telling me when these symptoms began?"
Historia de la enfermedad actual:
"Could you describe the pain in more detail? Is it constant or does it come and go? What makes it better or worse?"
Explicación del diagnóstico:
"Based on your symptoms and the test results, I believe you have [condition]. Let me explain what that means..."
Plan de tratamiento:

Comunicación de malas noticias (Bad News Protocol SPIKES): En situaciones difíciles, el protocolo SPIKES es el estándar en medicina anglófona:
- Setting: Lugar privado, sin interrupciones
- Perception: "What have you been told so far about your condition?"
- Invitation: "Would you like me to share the full results with you?"
- Knowledge: Dar la información en pequeños bloques, con pausas

¿Qué nivel de inglés necesita un médico para publicar en revistas científicas internacionales?
Para publicar en revistas indexadas como The Lancet, NEJM o JAMA, se necesita un nivel C1 como mínimo. Muchas revistas ofrecen servicios de corrección de estilo para no nativos. Lo más importante es dominar el registro científico: precisión, concisión y uso correcto de la voz pasiva y los tiempos verbales en investigación.
¿Cuál es el mejor examen de inglés para un médico que quiere trabajar en el extranjero?
El OET (Occupational English Test) es el más reconocido para médicos en Australia, Nueva Zelanda, Reino Unido e Irlanda. El IELTS Academic también es ampliamente aceptado. Para EE.UU., se requiere el USMLE en inglés, pero no existe un examen de inglés médico específico homologado por ECFMG.
